Application Process for Proof of Citizenship or Permanent Residency

To qualify for the Canadian Film or Video Production Tax Credit (CPTC), individual producers and key creative personnel must submit a copy of their proof of Canadian citizenship or permanent residency directly to CAVCO using the eSubmission application system. After reading the information below, you can access the secure application system by clicking on the eSubmission link in the left-hand menu.

All information retained by CAVCO is protected and confidential. CAVCO does not provide the CAVCO Personnel Number to anyone other than the individual to whom it is issued. Production companies must obtain this number from the individual .

Shortly after your application is received, you will be assigned a unique CAVCO Personnel Number that will be confirmed through a return email. You will need to provide this number to producers applying to the CPTC for productions in which you have a key creative role.

PLEASE NOTE: If someone (e.g. Agent, Production Company) is applying for the CAVCO Personnel Number on your behalf, they MUST do so by sending in a SIGNED application form (Form 01F17) with the appropriate documentation, by mail. You should NEVER allow someone to create an eSubmission account for you.

The following documents will be accepted as proof of Canadian citizenship or permanent residency (documents must not be obscured in any way and must reveal any registration number, including the reverse side of the document ):

For Canadian citizens:

  • Birth certificate from a Canadian province or territory (issued under the applicable Vital Statistics Act)
  • Canadian Certificate of Registration of Birth abroad (issued prior to 1977)
  • Certificate of Canadian Citizenship or Certificate of Naturalization (not commemorative issue)
  • Certificate of Indian Status
  • Registered Indian Record (must be certified)
  • Canadian Passport

For permanent residents:

  • Valid Permanent Resident Card (front and back).

Please DO NOT attach copies of your Social Insurance Number or any other identification cards or documents that are not listed above.

PLEASE NOTE: A valid Permanent Resident Card is the ONLY document CAVCO will accept to issue a CAVCO Personnel Number for permanent residents. Unfortunately we are unable to use Records of Landing or any other such document.

Canadian citizens do not need to resubmit copies of proofs of citizenship to CAVCO for future productions. However, since permanent residency status may expire over time, permanent residents need to resubmit their proof of permanent residency status to CAVCO when their permanent resident card expires. Permanent Residents who become Canadian citizens must submit their Citizenship Card to update their CAVCO Personnel Number.

CAVCO Personnel Numbers issued to permanent residents have a zero as the first number in the numerical portion of the number (e.g. ABCD0654). If a CAVCO Personnel Number provided to a producer identifies an individual as a permanent resident, the producer should ensure that the individual is able to demonstrate, with a valid permanent resident card, that he or she has permanent resident status at the time he or she provides services to the production.

To take advantage of PDF features:
